Terms of Service – Summer Kids Program

By submitting this form, I acknowledge and agree to the following:

1. Participation & Supervision
   I understand that my child(ren) will participate in various educational, spiritual, and recreational activities organized by the church. I give permission for my child(ren) to be under the supervision of designated teachers, volunteers, and church leaders during program hours.

2. Attendance & Punctuality
   I agree to ensure my child(ren) arrives on time and is picked up promptly. I understand that consistent attendance helps my child fully benefit from the program.

3. Medical Emergencies
   In the event of a medical emergency, I authorize the church staff to take appropriate action and contact emergency services if necessary. I acknowledge that I am responsible for any medical expenses incurred.

4. Behavior Expectations
   I will speak to my child(ren) about maintaining respectful behavior toward teachers, volunteers, and peers. I understand that the church reserves the right to address disruptive or unsafe behavior appropriately, including dismissal from the program if necessary.

5. Photo & Video Consent
   I give permission for my child(ren) to be photographed or recorded during the program for church-related purposes, including newsletters, social media, and promotional materials. (You may opt out by notifying the church directly.)

6. Liability Waiver
   I release the church and its representatives from liability for any injury or loss that may occur during the Summer Kids Program, except in cases of gross negligence.

7. Payment Policy
   I understand that registration is not complete until payment is received. Refunds are not guaranteed after the program begins unless due to exceptional circumstances.
